This work systematizes reflections on vocational problems from the perspective of professionals in vocational guidance, based on a process carried out in February 2020 with middle-level students. They constitute the first generation of graduates within their families. The determinants involved in the choice of a university career and the associated image were investigated from the following questions: “what kind of determinants influence the choice?” and “what occupational images relate to them?”. We worked from a qualitative, critical-hermeneutic analysis and applied open interviews and focus groups for the treatment of the dimensions. From the analysis, interpretation and systematization of the information, the following thematic nuclei were developed: economic determinants and university admission, image of degrees in health as tools for labor inclusion and social mobility, personal and family reparation, vocational guidance and primary health care (PHC). Finally, the importance of addressing vocational problems through public health policies is highlighted, from the comprehensive approach of PHC, especially in young people at educational risk.
